Appendix C: Request for confirmation of Nationality Status
Request for confirmation by Doctor/Dentist of Overseas Visitors nationality status to determine eligibility, or otherwise, to exemption from NHS charges for treatment. This Appendix should be read and completed in conjunction with Chapter 6 (NHSiS: Overseas Visitors, Management and Clinical Responsibilities) of this Manual.

Note: This patient is an overseas visitor as defined in the National Health Service (Charges to Overseas Visitors) (Scotland) Regulations 1989 1. Delete one of the statements at Section 2 below, to indicate whether in your opinion the diagnosis and any other treatment required by the patient is "treatment the need for which arose during the visit"2 as defined in the 1989 Regulations.
2. Section 2: To be completed by Doctor/Dentist (delete the inappropriate statement).

2.1. I am satisfied that the need for treatment arose during this patients visit to the UK, or

2.2. I am satisfied that the need for treatment did not arise during the patients visit to the UK.

1 Regulation 1(2) of the NHS (Charges to Overseas Visitors) (Scotland) 1989 (SI 1989 No 364), was amended by the NHS (Charges to Overseas Visitors) (Scotland) Amendment Regulations 1992 (SI 1992 No 411), the NHS (Charges to Overseas Visitors) (Scotland) Amendment Regulations 1994 (SI 1994 No 1770) and the NHS (Charges to Overseas Visitors) Amendment Regulations 1998 (SI 1998 No 251).
2 "Treatment the need for which arose during the visit" means diagnosis of symptoms or signs occurring for the first time after the visitor’s arrival in the UK, and any of the treatment which, in the opinion of a medical or dental practitioner employed by, or under contract with, a Health Board or as the case may be, NHS Trust, is required promptly for a condition which arose after the visitor’s arrival or became acutely exacerbated after arrival; or would be likely to become, acutely exacerbated without treatment.
