Key Elements of Joint Future

What are the key elements of Joint Future?

Joint Future requires partners to take holistic decisions on the organisation, management, financing and delivery of community care services for all care groups. The key elements are the foundation for sustaining delivery: Partnership Development, Improving Practice, Service Modernisation and Redesign, Whole Systems, and Evaluation. They were applied initially to older people's services from April 2003 and extended to all community care groups in April 2004. These elements help to deliver effective joint working between local partners in health, social care and housing on a number of levels.

Why have these key elements?

To deliver better results (i.e. better services and better outcomes) to meet the needs of those who require care and support and their families and to secure the best value for money.

How will these elements help to deliver our outcomes?

By enabling local agencies to work in partnership at all levels, to plan and deliver joint services across organisational boundaries, minimise bureaucracy and delegate decision making to front line staff.

When will we see results?

Getting these results means partners fully implementing the key elements of Joint Future across the board and ensuring continuous improvement based on mutually agreed local improvement targets.

The Joint Future Unit works with and assists partners to implement these key elements by targeting particular areas of activity.
