In November 2006 a wide group of stakeholders convened to develop a set of measures for Community Care which moved away from monitoring inputs, processes and outputs, and instead focussed on pinpointing Outcomes which are important for people who use Community Care services, and their carers. Since then, these have been continually refined and developed by the Outcomes Design Authority and seven 'Early Implementer' partnerships, engaged in piloting various elements of the Community Care Outcomes Framework.

A letter was issued in December 2008 outlining these developments and we are now very pleased to present the final definitions of the measures, for use in 2008-9 and beyond.

A recent round of visits to local health and community care partnerships showed that there is a significant appetite to adopt these measures, either to assist with formal Single Outcome Agreement (SOA) reporting, where relevant, or 'below the waterline' to feed in to local performance management arrangements.

The Midlothian partnership are currently developing proposals for a Community Care Benchmarking Network which will allow local partnerships to compare their performance against these measures. An invitation to this group will go out to all partnerships across Scotland in early 2009.
