Audit currently being completed to evaluate existing activity and to review plan, available December 2005

What data did you collect and how often was it monitored?

1. Statistical data relating to patients delayed

2. Devised monitoring tool to measure length of stay

3. This data is monitored on a weekly basis internally and on a monthly basis for ISD

4. Finance and Performance reports on delayed discharge linking financial data with bed occupancy and emergency admission rates
