The Good Practice Resource provides examples of good practice in relation to delayed discharge. It profiles innovative initiatives and mainstream activities which aim to reduce the number of people whose discharge from hospital is delayed. Further examples of good practice in hospital discharge can be found in the Princess Royal Trust for Carers' internet pages.

We need your help with this. If the good practice resource is to be of value to people it has to be kept current and credible. We rely on colleagues working in the fields of health, housing and social care to let us know about services or arrangements that they have put in place that are making an impact on the problem of delayed discharges.

We would also like to hear about the things that did not go as well as expected. These examples may provide transferable lessons on what to avoid and how to manage such a situation.
