2. JOB PURPOSE

The post holder as part of a generic/generalist community health nursing team will contribute to the national health service agenda through the application of research evidence based practice to enable patients and the public to improve and sustain their health and well being provide care and promote health and well being. The post holder will have responsibilities delegated by the Community Health Nurse (CHN) which will include participating in the assessment of children and young people, adults, families and carers, implementing planned programmes of care and reviewing and evaluating the effectiveness of nursing interventions.

This will be achieved by working within the seven core elements of nursing in the community; Working directly with individuals and carers, adopting public health approaches to protecting the public, co-ordinating services, supporting self care, multi-disciplinary and multi-agency team working, meeting the health needs of communities and supporting anticipatory care. It will be further enhanced by using a proactive approach to promoting health and wellbeing, detection and prevention of vulnerability, preventing illness, reacting and responding to existing or potential ill health issues and contributing to reduction in unplanned hospital admissions.

The post holder will be accountable and responsible to the Community Health Nurse and will deliver

planned programmes of care to individuals, families and communities within the area in a variety of settings.

The post holder will supervise support staff and will contribute to the development of an integrated care/team working approach.

The post-holder will collaborate with a wide range of statutory and voluntary organisations to maximise the nursing contribution in health improvement, planning and delivery of care to individuals, families and communities.

The main functions and objectives of the Community Health Nursing service are to:

Ø To contribute to the seven core elements of nursing in the community

Ø Assess, anticipate, identify, promote and maintain the health needs of the local population in partnership with colleagues, individuals, families, carers and the local community in a variety of settings.

Ø Collaboratively plan with stakeholders and implement and deliver care, treatment and early interventions as assessed.

Ø Evaluate outcomes of planned programmes of care and support

This will be addressed by a public health approach to the health and well being of children and young people,

adults, families, carers and communities within a robust clinical governance framework in line with local and national

priorities and standards.

6. KEY RESULT AREAS

Leadership and Management:

Assists the Community Health Nurse in the provision of evidence based quality nursing care to a defined population through assessment, planning, implementing and evaluating programmes of care for children and young people, adults, families, carers and communities.

Assists the CHN to plan and implement and evaluate care for children, young people, adults, carers and families who will benefit from clinical case management due to long term conditions, palliative and terminal care and complex needs requiring individualised packages of care.

Supervises support workers in providing safe, efficient and effective community health nursing care

Work collaboratively with multi-disciplinary and multi agency team to promote health, prevent illness, protect the public and reduce health inequalities.

Participates in individual appraisal and PDP of self and support workers in line with the KSF framework and address practice improvement issues as appropriate, Participates in clinical supervision and promotes reflective practice

Mentors and educates learners within the community nursing setting.

Health Needs Assessment

Works collaboratively with the public health improvement team within the organisation/CHP in the health needs assessment of the population.

Support a range of intervention that contributes to improving the health and wellbeing of individuals, families and communities.

Contributes as part of the CHN team to the reduction of unplanned hospital admissions and facilitates early discharge by anticipating care needs, facilitating and providing self care initiatives and working in partnership with carers and other stakeholders.

Contributes to improving physical and mental health and well being, promotes capacity and capability for children and young people, adults, families and carers to self care

Practice Development:

Contributes to own development of clinical practice interventions based on the most recent evidence available ensuring the findings are disseminated and incorporated into nursing practice.


Demonstrates and delivers nursing skills, knowledge and competence in clinical practice and public health interventions.

Acts as role model to other (junior) staff in the inclusion of best practice.

Participates in the development of new evidence based practice to promote health and well being.

Clinical Governance

Work within the framework of NHS Board policy e.g. health protection policy, health and safety, assessment of risk and management of staff and patients, critical incident reporting,

Implements evidence based practice as applicable to own area of practice

Contributes to a culture that improves quality by promoting care and compassion towards individuals, patients and communities by ensuring continuous review and development of practice and service delivery. This will be achieved by clinical audit, patient feedback and reflection on practice by self and other members of the team.

Comply, maintain and promote standards of record keeping in accordance with NMC regulatory body, national legislation and local standards.

Facilitate effective communication with multi-professional and multi-agency team.

Patient and Public Experience:

Engage children and young people, adult's family's carers and communities in decision making in an inclusive non-judgemental approach, recognising equality and respect for cultural diversity.

Supports the CHN to provide the best care, treatment and support for health improvement based on the most recent evidence available.

Responsible for signposting and referring individuals, families, carers and communities to the most appropriate services and agencies.

Identifies opportunities to develop high quality community nursing services in collaboration with the CHN.

Have the knowledge and understanding of local Complaints Procedures and their role in responding to patients, individuals and families concerns.

8. ASSIGNMENT AND REVIEW OF WORK

The post holder will be responsible for the day-to day prioritisation and organisation of their work as delegated by the CHN and the delegation of workload as appropriate to support workers.

The post holder as part of the CHN team will work within the broad principles of the seven elements of community health nursing practice to respond to the needs of the local population, taking into account the strategic priorities of the NHS Board and CHP.

The Community Health Nurse will be responsible for agreeing objectives, reviewing performance and supporting professional development.

Workload will be delegated by the CHN and generated by -GP, hospital teams, patient demand, self referral, other health care professionals local authority, and other agencies based in the community including the voluntary sector
