2. JOB PURPOSE

To contribute to the work of community health nursing team (CHNT) in the provision of holistic, evidence based care to children and young people, adults, families, carers and communities.

To enable and empower individuals, families and communities to engage in healthy lifestyles.

To work in collaboration with other key stakeholders involved in the delivery of care in the community.

Work within CHN Team under supervision of registered nurse

3. DIMENSIONS

The post holder will be accountable and responsible to the community staff nurse.

The post- holder will contribute to the delivery of planned programmes of care to children and young people, adults, families, carers and communities within the area in a variety of settings as designated by the CHN team.

5. ROLE OF DEPARTMENT

The main functions and objectives of the Community Health Nursing service are to:

Ø To contribute to the seven core elements of nursing in the community

Ø Assess, anticipate, identify, promote and maintain the health needs of the local population in partnership with colleagues, individuals, families, carers and the local community in a variety of settings.

Ø Collaboratively plan with stakeholders and implement and deliver care, treatment and early interventions as assessed.

Ø Evaluate outcomes of planned programmes of care and support

This will be addressed by a public health approach to the health and well being of children and young people,

adults, families, carers and communities within a robust clinical governance framework in line with local and national

priorities and standards.

6. KEY RESULT AREAS

The Health care support worker will:

  • Work across the core elements of nursing in the community in collaboration with CHN team
  • Work directly with children and young people, adults, families and carers
  • Support the public health approach to protecting the public including contributing to the protection of vulnerable
  • Children, young people and adults
  • Support the co-ordination of services as part of the CHN team, providing nursing care within agreed standards and consistent with safe practice
  • Support individuals and their carers in their capacity to self care
  • Recognise and work within the boundaries of practice, respecting the roles and responsibilities of other health and social care providers.
  • Refer to the CHN team for advice when required or knowledge is out with their sphere of competence
  • Practice within a legal framework encompassing statutory, organisational policy and guidelines
  • Identify own educational, training and support needs within framework of regular supervision
  • Observe confidentiality in accordance with NHS Board policy
  • Assist in supplying and maintaining materials and equipment
  • Provide practical assistance to individuals in carrying out everyday living tasks
  • Be able to communicate progress and/ or any issues that may arise to the CHN/ Staff Nurse

7a. EQUIPMENT AND MACHINERY

Sphygs for blood pressure recording and syringes if trained

7b. SYSTEMS

Under supervision of Community Staff Nurse contribute to the following

  • Electronic and paper record systems
  • Activity recording systems
  • Others as and when required

8. ASSIGNMENT AND REVIEW OF WORK

Workload will be generated by the Community Health Nurse in accordance with the needs of the caseload.

The post holder will work within the broad principles of the seven elements of practice to respond to the needs of the local population

The post holder will be expected to provide clear reports back to the CHN/ Staff Nurse

The role of the Health care support worker is to deliver care as prescribed by the registered nurse

  • Working within a potentially large CHN Team
  • Working across a range of ages

Clients, patients, carers, the nursing and primary care team, other healthcare professionals, members of staff from other agencies, administrative staff, education staff

Must have good communication skills to be able to interact with and report back to a wide variety of clients and professionals
