The Improvement & Support Team (IST) - National Improvement Programmes 2008 - 2011

The Improvement and Support Team (IST) now has three new National Improvement Programmes supporting the commitments in " Better Health, Better Care" and the delivery of refreshed HEAT Targets.

These new programmes will build on the experience and learning gained over the last 3 years from the previous Unscheduled Care, Diagnostics, Eye Care and Planned Care Programmes which ended in March 2008.

New Programmes:

18 Weeks Service Redesign and Transformation Programme.

Delivery of this programme will require a focus on whole patient pathway redesign across the following areas:

The Improvement and Support Team (IST) will support NHSScotland to deliver and sustain the 18 Weeks referral to treatment standard through this three year national improvement programme engaging with every NHS board. This 3 year programme is being developed in partnership with NHS Lothian.

This is part of an integrated programme of delivery which was launched on February 6, 2008 by the Cabinet Secretary for Health and Wellbeing. www.18weeks.scot.nhs.uk

The 18 Weeks Service Redesign and Transformation Programme builds on knowledge and experience of the application of tools and techniques to improve clinical systems and processes. This experience of improving management of demand and capacity, improving patient flow by reducing bottlenecks and using small scale rapid cycles of change to make incremental and sustainable improvement, will provide a solid basis to move forward.

The programme will also build on the appropriate approaches to behavioural change management that will be required to deliver service transformation of the scope and scale which the programme requires.

The programme will be supported by the implementation of key high impact changes: improve referral and diagnostics, actively manage admission to hospital, treat day surgery as the norm, actively manage discharge and length of stay, actively manage follow-up.

Long Term Conditions Collaborative

Long Term Conditions Collaborative Programme, led by Susan Bishop, National Programme Manager.

The Long Term Conditions Collaborative was officially launched in conjunction with the Mental Health Collaborative Programme on April 24, 2008 by the Chief Medical Officer.

This is a 3 year National Programme which is being led by IST in partnership with NHS Tayside.

The programme's focus is across the following 3 work streams:
Self Management
Specialist Care
Complex Care

The programme incorporates improvement work led by the Scottish Primary Care Collaborative, retaining the scope and funding agreed for this work.

Mental Health Collaborative Programme

Mental Health Collaborative Programme, led by Ruth Glassborow

The Mental Health Collaborative was officially launched in conjunction with the Long Term Conditions Collaborative Programme by the Chief Medical Officer at the Dunblane Hydro on April 24, 2008.

This is a 3 year National Programme which is being led by IST in partnership with NHS Greater Glasgow and Clyde.

The Programme supports NHSScotland in the implementation of tools and techniques for Clinical Systems Improvement. The aim is to deliver improved patient centred services with the focus on reducing the prescribing of antidepressants and the number of re-admissions.

Service, Innovation & Transformation Programme

The Service Innovation and Transformation Programme supports the delivery of the improvement programmes by applying innovation to deliver service change.

The programme is currently working in partnership with NHS Tayside to develop an approach to service innovation that compliments and enhances the existing service improvement methodology.

This is being undertaken over the summer of 2008 with the opportunity to hear about and comment on team developments in the Autumn.

Core Improvement Skills

Core Improvement Skills training aims to support NHS Staff to lead improvement work effectively as well as utilising core improvement tools and techniques and creating opportunities for improvement.

Each day is designed to stand alone or they can be booked together as a practitioner level improvement course. More information on dates and courses is available in the flyer and on our Events Webpage.

NHS Staff now have direct access to our IST Online Continuous Improvement Toolkits.

Improvement Master Class Programme Web Casts

These Full Day Master Classes were targeted at Senior Management and Senior Improvement Leads and were aimed at providing challenging, thought provoking discussion and learning opportunities for developing and supporting continuous improvement.

Webcast recordings are still available of the day for each of the Master Classes and are available for viewing together with all the support materials.

Continuous Improvement Toolkit

The IST Online Continuous Improvement Toolkit has been designed by the Improvement Support Team to support managers and staff involved to improve clinical systems and processes. The Toolkit provides users with a ready-made resource pack full of practical support and guidance to help facilitate and lead improvement.
