Forms for use in Assured and Short Assured Tenancies

If you have an assured tenancy or short assured tenancy or want to set one up, you have to use the official forms for certain purposes. These forms are provided here to print out. You can also obtain paper copies from the Scottish Government's Private Sector Housing Team.

  • AT 1(L) Notice under section 17(2) of the Housing (Scotland) Act 1988 proposing terms of a statutory assured tenancy different from the terms of the former tenancy (for use by landlords only)
  • AT 1(T) Notice under section 17(2) of the Housing (Scotland) Act 1988 proposing terms of a statutory assured tenancy different from the terms of the former tenancy (for use by tenants only)
  • AT 2 Notice under section 24(1) of the Housing (Scotland) Act 1988 of an increase of rent under an assured tenancy.
  • AT 3(L) Application by a landlord to a Rent Assessment Committee for a determination of the terms of a statutory assured tenancy and, if appropriate, rent for that tenancy under Section 17(3) of the Housing (Scotland) Act 1988.
  • AT 3(T) Application by a tenant to a Rent Assessment Committee for a determination of the terms of a statutory assured tenancy and, if appropriate, rent for that tenancy under Section 17(3) of the Housing (Scotland) Act 1988.
  • AT 4 Application by a tenant to a rent assessment committee for a determination of rent. Under sections 24(3) and 34(1) of the Housing (Scotland) Act 1988.
  • AT 5 Notice under Section 32 of the Housing (Scotland) Act 1988 to be served on a prospective tenant of a short assured tenancy. Form AT5 must be issued to tenants before they sign their lease to create a short assured tenancy. Prescribed document
  • AT 6 Notice under Section 19 of the Housing (Scotland) Act 1988 of intention to raise proceedings for possession.
  • AT 7 Notice under Section 32(4) of the Housing (Scotland) Act 1988 that a new or continuing tenancy is not to be a short assured tenancy.
