Authors' acknowledgements

Our first thank you is to the 11,472 men, women and children who gave up their time voluntarily to take part in this survey in 2003/4 and welcomed our interviewers and nurses into their homes.

We would also like to thank the interviewers and nurses who worked on the original data collection. We owe a huge debt of gratitude for the dedication and professionalism they applied to their work.

A number of research colleagues assisted with the secondary analysis presented in this report and with producing the final version of the report, in particular: Catriona Webster, Claire Deverill, Liz Fuller and Kelly Ward.

Finally, special thanks are due to Sonnda Catto at NHS Health Scotland for conceptualisation, initial design, specification and commissioning of the project and her considerable input at the report writing stage.

Thanks are also due to the Scottish Government Health Analytical Services Division, in particular Julie Ramsay for her support at all stages of the project, and Iain MacAllister and Emma Stevens for their input at the reporting stage.
