CLAIMING FREE PRESCRIPTIONS:
If you don't have to pay, put a cross on the back of your prescription form in the first box in Part 1 that applies to you. Then complete and sign Part 3. You may be asked to show proof of your entitlement to free prescriptions - read here and the tables for more information about this.
