1. The interviewers also recorded that some people gave other reasons, which were not identified separately in the interviewers' code-lists. Excluding those who gave no reason, or who were not asked for the reasons (with effect from 2005, reasons are only sought in the case of a sub-sample)
2. Most of the cases for which the SHS contractors coded unspecified "other" reasons are from interviews which were conducted in 2001, 2002 and 2003
