INFORMATION ABOUT CHECKS OF ENTITLEMENT
If you are claiming free treatment or prescriptions, or using an optical voucher, you should show proof of why you don't have to pay or why you can use a voucher, e.g. a benefit book, birth certificate or NHS exemption certificate. For more information about this read the tables.
If you cannot show proof, you will get your NHS treatment or prescription free, or be able to use your optical voucher. However, your treatment form or prescription form will be marked "no evidence seen" and your entitlement may be checked later.
Please Note:
Patients exempt from NHS prescription charges on age grounds need not produce evidence where their date of birth is specified in an electronic prescription form, or printed by a computer on a non-electronic prescription form.
However, patients exempt on age grounds will still be required to sign the patient declaration on the reverse of the prescription form as confirmation that the medicine or appliance ordered was dispensed.
NHSScotland Counter Fraud Services will check claims for:
- Free prescriptions.
- Full or partial help with NHS dental treatment.
- Vouchers towards the cost of glasses or contact lenses.
