CLAIMING FREE PRESCRIPTIONS:
If you don't have to pay, put a cross on the back of your prescription form in the first box in Part 1 that applies to you. Then complete and sign Part 3. You may be asked to show proof of your entitlement to free prescriptions and the tables for more information about this.
