Types of intervention and links to other services

Links to some of these services may be found on the self-help toolkit issued in March 2006. However, it is the responsibility of local areas to keep this list up-to-date.
The practitioner will work with the patient in a collaborative, facilitative manner to help the patient clarify their needs and may refer them to other agencies such as voluntary organisations, local authority leisure facilities, vetted web-sites and many more.
Strong links should be made with these groups and in order to achieve appropriate signposting to these services, it is essential that this is done with full knowledge of the range and quality of services offered by each local organisation. This includes the need for quality information resources about local organisations with the need for regular updating of contact details.
It is important that locally provided non-statutory services are reviewed and updated on a monthly basis.
