APPENDIX 5 EXPERIENCE QUESTIONNAIRE

Centre for Disaster Management - Scottish Executive Capability Mapping Project
We are currently collecting information on behalf of the Scottish Executive as part of their capability mapping project. Part of the data we need to collect relates to emergency planners (and others involved with major emergencies/disaster) from all sectors, their responsibilities and experience. In order to do this the Scottish Branch of the EPS has very kindly allowed us to include this questionnaire in your delegate packs.
Unfortunately, colleagues from the Scottish Executive are taking part in a national exercise at the same time at this year's conference. However, if you have any questions on the research, please contact:

We would be very grateful if you could take 5 to 10 minutes to complete the questionnaire and then place it in the box provided at the registration desk.
The first 2 questions ask for personal information about yourself, we wish to emphasise that you are not obliged to complete this section and can remain anonymous if you wish. However we are looking to do some follow up work with regard to local level emergency planning/disaster management and would ask that those of you who wish to participate in subsequent focus groups or in depth interviews complete this section. We will then be in touch with you at a later date when we are arranging these events.
We very much hope that you choose to complete this questionnaire as the information you provide is very important to the development of Scotland's future resilience and we are sure will prove invaluable.
