Update from Joint Improvement Team
After a hectic, but rewarding, first year, the Joint Improvement Team ( JIT) is working with 26 of the 32 health and social care partnerships across Scotland in a variety of ways, from developmental practice work to intensive support programmes. We're about half way through our intensive programmes with Argyll and Bute, Edinburgh, and Western Isles, and we're implementing a new programme with Inverclyde. We're working closely with Scottish Borders on our housing programme, Forth Valley on Commissioning, Ayrshire and Arran partnerships on Intermediate Care, and Lanarkshire partnerships on Managed Care Networks.
We held a conference on 19 June in Clydebank to promote our Transport with Care programme - in conjunction with the Scottish Ambulance Service - at which Health Minister Andy Kerr emphasised that transport is an integral part of the health service, and needs to be recognised as a key part of the patient journey and recovery. We're currently undertaking a data-gathering exercise with a small group of partnerships to inform our Stage 2 EGF bid, which will be submitted by the end of August.
We've secured an £8m development fund for our Telecare programme, a share of which will be available to all partnerships. We're currently working on guidance that will provide more information about the programme, the benefits and efficiencies of Telecare, the allocation of funds, and how to apply for a share to support local Telecare initiatives.
We're also making progress in our work on Strategic Joint Commissioning, Governance, supporting evidence-based commissioning and practice for Learning Disability services, Managed Care Network developmental work, and Intermediate Care evaluation and Learning Networks.
