Started in selected GP practices, then expanded to target population of 80,000:

  • A range of interventions available through Doing Well team staff
  • Secondary care expertise available to GPs through Doing Well Team

  • Public involvement and community development element introduced 2005.
  • Public and community awareness raising. Tier 1
