How To Complete This Diary

This research project seeks to find out about people's experiences of using services in your area. By keeping this diary you will be helping yourself (or the person who is attending the focus group) to talk about a wide range of experiences.

The diary contains 7 pages, one for every day of the week. Each page is the same and asks questions about the services that you have used that day or would have liked to use. We would like you to answer these questions every day in the week leading up to the group.

If you are not the group attendee, when you have completed the diary on day 7, please give it to the person who is attending the focus group for them to read and take to the group.
