Who should conduct the audit?
Although the person conducting the audit is at the discretion of the unit involved, it is envisaged that they will either be a member of the Infection Control team or a senior member of the Endoscopy Unit.
Who should give the answers to the audit questions?
In general, the person answering the questions should be the member of staff directly involved in carrying out the procedures being audited. This may include unit nurses or members of the Estates department.
Completing an Audit Questionnaire
Questionnaires should be tagged as being complete once the auditor is happy that all the information is completed and correct. Once a questionnaire is complete, no further amendments to the data are allowed, although the data collected can be viewed on screen.
The completion process is accomplished from the Available Questionnaires per Section screen of the ADAT program (See:Available Questionnaires per Section). Click on the COMPLETE command button to start the process. You will be prompted as to whether you wish to complete and sign off the questionnaire, with the warning that further amendments to the data will not be available once this is completed. Selecting YES at this stage will bring up a final confirmation dialog before the questionnaire is finally and completely signed off.
This action is not reversible in any way. There is no administrative function that will allow users with that security level to reverse the process, so it is important that the auditor is completely happy with the data before closing off the questionnaire.
There are no requirements within the program that all questionnaires be completed before moving from a section or moving to another module or audit. The completion function is made available so that entered data may be secured against accidental editing at a later date.
Please Note: It is only possible to complete a questionnaire when the status is flagged as Incomplete. (See:Questionnaire Status). Deferred questionnaires or those that are Awaiting Initial Data Entry cannot be completed.
