Draft New Movement Document

Notes:

  1. This modified example shows the A5 size currently in use and contains all the details that must be provided under EC Regulation 21/2004.
  2. If industry wishes, the new version can be increased to A4 size and more space allocated to specific areas. Are there any non-official details that you want included on an A4 version?
  3. All details above the grey line at 2. should be completed by the keeper at the departure location before the animals move. Either the destination CPH or address must be stated.
  4. Haulage details @ 3. must be completed before the journey commences - the driver has ultimate responsibility for ensuring this section is completed.
  5. Keeper at destination location must check details in Section 1 are correct and add any missing details e.g. CPH. Then complete the required details at 2. Send to SAMU within 3 days
  6. Currently 2 copies of each MD are provided. Would it be beneficial to have a third copy - either for sending to SAMU or for the haulier's records?
  7. We intend printing guidance on how to complete and use the MDs on the inside cover of the folder that holds the pre-printed MDs that SEERAD will provide. Contact details for SAMU will also be provided.
