Footnotes
  1. Response rates to telephone surveys can be calculated in a number of ways. In the method used here, it was assumed that all telephone numbers from which a ringing tone was obtained, but no answer, was a valid and 'live' number. In other methods, numbers which persistently give no answer are considered to be out of scope, resulting in higher response rates.
