CARER'S QUESTIONNAIRE

About this questionnaire

What we mean by "carer"

For convenience, we have used the word "carer" in this questionnaire to refer to people who help the person being assessed, on an unpaid basis.

The following is a definition of the name "carers" by Carers UK (the organisation which has worked to convince government of the importance of family members and friends in providing unpaid support and help):

"Carers look after family, partners or friends in need of help because they are ill, frail or have a disability. The care they provide is unpaid."

Carers UK recognise that many people who look after family or friends in this way do not think of themselves as "carers".

There may be more than one person who helps the person being assessed in this way.

How to fill in this questionnaire

The questionnaire has 2 sections:

The first is about getting ready for the assessment meeting(s)

The second is about how the assessment meetings were carried out.

Please answer all the questions that you think are relevant to you.

The questions have tick boxes asking you to say 'yes' or 'no'. If you feel your answer should be 'yes' and 'no' please explain this in the 'Your Comments' box. We would also welcome any other comments
