Dear Householder,

Important Survey in Scotland

This letter is being sent to ask for your help with an important survey being conducted across Scotland. Your address has been randomly selected, on the basis of your postcode, to take part in the survey. It will monitor the Scottish Executive's work to improve the nation's health and well-being and reduce health inequalities. The survey is conducted every two years and is designed to explore what the people of Scotland think about health and quality of life issues. The questionnaire opens with a section on general health and well-being, before focusing more specifically on issues relating to mental health. We hope you will agree to a face-to-face interview lasting around 30 minutes.

The survey questionnaire has been designed by a group of experts and the interviews are being carried out by MORI Scotland. If you agree to talk to the MORI researcher, everything you say will be treated in total confidence. No data will be seen by anyone outside the survey team. Naturally you can refuse to answer any questions you do not wish to respond to and you will be free to stop the interview at any time you wish. If you have any queries about the survey, you can leave a message for Sara Davidson at MORI on free phone 0800 328 9834 and she or a colleague will get back to you. If you call, please quote your reference at the top right of the letter.

In order to ensure that the survey covers a wide range of experiences, MORI will try to ensure that any particular needs you have (relating to language, sensory abilities etc) are accommodated in the interview. The Scottish Executive is committed to policies aimed at improving the health of the people of Scotland. The (anonymised) results of the survey will be published in due course by the Scottish Executive so, by agreeing to participate in the survey, you will be playing an important part in the work. We hope you will agree to be interviewed. An interviewer from MORI Scotland will be visiting you soon.
