D. TREATMENT AND REHABILITATION

D1. INTEGRATED CARE

Progress in April 2003-March 2004

  • We have continued to produce Integrated Care Pathway Guides with the support of a Working Groups drawn from health, social work, and the voluntary sector. In July 2003 we published 'Developing Integrated Care Pathways' (Guide 2); in January 2004, 'Implementing Integrated Care Pathways' (Guide 3). Guide 4, 'Analysis and Review' will be published in April 2004.

  • In January 2004, we published 'A Practical Guide to Needs Assessment for Drug Users' which sets out the rationale for the strategic process of needs assessment, identifies the key components and provides a step-by-step guide. The guide was developed following a telephone survey of DAATs and a review of the literature on needs assessment. A working group drawn from key agencies supported this work.

  • We have taken forward evidence gathering to support the development of a Guide to Advocacy for Drug Users. In the course of 2003-04, we reviewed research and policy documents on the principles and practice of advocacy, carried out a survey of advocacy services, held a consultation seminar and commissioned Scottish Drugs Forum to conduct a focus group study with service users.

  • We took forward our commitment to support the development of information sharing. We set up a short-life working group and carried out a DAAT telephone survey to find out more about local information sharing developments and support needs. The EIU has now commissioned an external consultant to provide facilitation support, in the form of workshops, to all DAATs who wish them. Workshops will be tailored to meet local needs, recognising that areas are at different stages of development. The first 3 workshops were held in March 2004 in Dumfries and Galloway, Fife and Highland. The EIU is also working with DAATs to encourage publication of local information sharing materials on the DAAT pages of the Drug Misuse Information Scotland website, to assist the exchange of practice examples between areas.

  • In conjunction with Greater Glasgow NHS Board, Greater Glasgow Primary Care NHS Trust and Glasgow City Council, we continued to support the evaluation of the working of two new integrated Community Addiction Teams.

  • We agreed with the Joint Future Unit and the DAT Association to promote and support the development of integrated care, both at strategic and operational level, by offering joint, local workshops tailored to local needs and circumstances. We have organised one workshop in Midlothian ( February 2004) which has provided useful lessons for the future. We have set up a working group from NHS, social work and the voluntary sector to help us devise the framework, content and format for the workshops.

  • We engaged in preliminary discussions with DAATs about the issues that impact on the development of integrated care in rural and remote areas.

Plans for April 2004-March 2005

  • We will continue to promote and support the implementation of integrated care.

  • We will publish 4 Integrated Care Pathway Guides: Community Detoxification ( June 2004); Care of People with Drug problems in Psychiatric Settings ( August 2004); Drug Misuse in Pregnancy ( October 2004); and Care of People with Drug Problems in Acute Medical and Surgical Settings ( December 2004). We will collaborate with the Joint Future Unit on an ICP Guide to Single Shared Assessment ( October 2004) . In the autumn, we will hold 3 seminars on Integrated Care Pathways.

  • We will publish a Guide to Advocacy for Drug Users in June 2004.

  • We will offer local workshops to DAATs and partner agencies on integrated care, in partnership with the Joint Future Unit and the DAT Association.

  • We will evaluate the first 3 information sharing workshops to inform the design of the subsequent workshops to be held throughout the year.

  • We also plan to hold regional seminars on needs assessment, Integrated Care Pathways and advocacy. However, in line with our focus on facilitation and support we would also be willing to collaborate with DAATS and other local agencies on running local events.

  • We will examine the issues in developing integrated care in rural and remote areas and produce a document on effective approaches to the commissioning, managing and delivery of drug services in rural and remote areas by December 2004.

D2. EFFECTIVENESS OF TREATMENT

Progress in April 2003-March 2004

  • We undertook a review of the literature on the effectiveness of residential rehabilitation and detoxification services for drug users, and the findings were used to inform the review of treatment and rehabilitation services undertaken by the Scottish Executive Substance Misuse Division.

  • At the request of the Deputy Justice Minister, we commissioned a National Investigation into Drug-related Deaths ( December 2003). This Investigation will undertake a detailed examination of the social and clinical circumstances surrounding all drug-related deaths in Scotland in 2003.

Plans for April 2004-March 2005

  • We will publish a review of residential detoxification and rehabilitation services for drug users in Scotland ( June 2004).

  • In the course of the year we will commission or co-fund further research on

    - counselling and other psycho-social interventions
    - effectiveness of residential rehabilitation
    - the effectiveness of buprenorphine

  • We will publish the findings from the National Investigation into Drug-related Deaths ( December 2004), and a report comparing the circumstances of Scottish drug-related deaths with drug-related deaths in London in the same period ( March 2005).

D3. HEPATITIS C

Progress in April 2003 - March 2004

  • We commissioned an evaluation of a Hepatitis C peer education project called C-Level ( May 2003). The aim of the evaluation is to explore the views of staff, clients and funders of the C-Level project, to describe the nature and range of services offered by the project, and to examine the experience of recruiting, training, and supporting the peer educators.

  • We commissioned ( October 2003) an evaluation of the impact of changes in the Lord Advocate's Guidance on the number of needles and syringes that can be issued at one time.

  • We published the report of the research on Examining the Injecting Practices of Injecting Drug Users ( February 2004) and held a dissemination seminar in March 2004.

  • We commissioned the second phase of the injecting practices research in November 2003. This is a laboratory based study of safety, risks and outcomes from the use of different forms of injecting paraphernalia among injecting drug users. This study is due to be completed in ( February 2006).

  • We will publish

    - report of evaluation of Lord Advocate's Guidance in February 2005.

D4. SPS TRANSITIONAL CARE

Progress in April 2003 - March 2004

  • We continued to manage the evaluation of the Scottish Prison Service's Transitional Care programme.

Plans for April 2004 - March 2005

  • We will publish interim findings from the SPS Transitional Care evaluation ( April, June and November 2004).

D5. TRAINING AND EMPLOYMENT

Progress in April 2003- March 2004

  • We published the Moving On Update in October 2003. The Update reflects more recent developments in employability provision and examines the importance of the role of employers and the requirements of local labour markets. We developed the Update with the support of a working group.

Plans for April 2003-March 2004

  • We plan to develop and offer locally based workshops on the development of employability provision for drug users. The workshops will also reflect recent developments in the Healthy Working Lives strategy being developed by Substance Misuse Division as part of the implementation of the Health Improvement Challenge.

D6. PSYCHOSTIMULANTS

Progress in April 2003- March 2004

  • We have continued to support the development of the pilot service for psychostimulant drug users in Aberdeen. Development of the pilot service (the 'Incite' project) has been informed by needs assessment work conducted by the pilot project staff, in partnership with EIU, and the service began operating in March 2004.

Plans for April 2004 -March 2005

  • We will support the on-going monitoring and evaluation of the pilot psychostimulant service in Aberdeen (the 'Incite' project). An external evaluation of the service will be commissioned in June 2004.

D7. CO-MORBIDITY

Progress in April 2003- March 2004

  • We conducted an in-house review of existing literature. We also supported the work of the SACAM/SACDM Multi-Mental Health Working Group.

  • We commissioned a study on the provision of health & social care services for people with co-existing mental health and substance use problems in Scotland in November 2003.

Plans for April 2004 -March 2005

  • The study on existing service provision in Scotland for people in this client group is due to be published in March 2005. This may lead to further research.
