Interim Guidelines for Smallpox Response and Management in Scotland in the Post-Eradication era

Appendix 19: training of healthcare staff
  1. Most training needs will arise from the contents of this plan. It will not be possible to define precise training needs until the final plan is agreed

  2. The following therefore gives a general outlines to provide a framework of training needs as they relate to what can at present be identified as service requirements.

Table 1: primary and secondary care level

1.1 Diagnosis: Alert level 0

  • Training on recognition of smallpox and distinguishing smallpox from more common conditions

1.2 Treatment: Alert Level 1 or 2

1.3 Contact management: Alert level 1 or 2

Ensuring that contact vaccination and monitoring is undertaken according to the smallpox plan

  • Awareness of classification of contacts including criteria for reclassification of contacts who develop symptoms.

  • Awareness of the measures to be taken in relation to contacts.

  • Awareness of the role of NSOCC and LSOCC

  • Information on who to contact for advice and support in dealing with contacts.

CPHMs (CD&EH) and other Public Health staff

Identification and appropriate action in relation to cases and contacts

Training to ensure that all CPHMs (CD&EH) and public health staff are familiar with the smallpox plan and the action that they will be required to take in the event of a case(s).

  • Infection control nurses in hospitals

Ensuring that possible cases are not inappropriately exposed to risk from probable and confirmed cases

Training on appropriate isolation/vaccination arrangements for possible probable and confirmed cases at smallpox centres

  • Physicians

  • Occupational health staff

Protection of healthcare workers

Training on vaccination requirements and protection requirements for healthcare workers dealing with possible, probable and confirmed cases

1.4 Vaccination: Alert level 1 or 2

Competence in rapidly training large numbers of nurses in smallpox vaccine administration

Training the trainer approach in all aspects of smallpox vaccination, using materials, videos etc

1.7 Mass media

Ensure accurate non alarmist communication with the public to achieve compliance with the measures in the smallpox plan

Training on the importance of observing the appropriate channels of communication in the event of cases or suspected cases

Training to ensure that CPHMs (CD&EH) working methods and responses are consistent with agreed Health and Emergency services procedures

2.2 Outbreak management: Alert level 0

  • Ability to isolate case and perform effective ring vaccination programme to protect the wider community

  • Ability to react rapidly and effectively

  • Training on vaccination strategies designed to protect the wider population in relation to smallpox.

  • Awareness training on what should be in place at local level and how these arrangements should link to regional, Scottish and UK plans
