ANNEX H What Young People Should Expect When Leaving Care

Entitlement
You, your family and your carers will be made aware of what throughcare and aftercare support and services you will be entitled to.

Prior to leaving care, you will be given clear and understandable information about your rights, available services and where to go for on-going support and advice.

Throughcare Preparation
You will be sensitively supported and encouraged to plan for the future, well in advance of the time that you will actually leave care.

Your care plan and reviews will look at your future throughcare and aftercare needs, and your views will be taken into account.

Before leaving care, you will be able to develop adult living skills, which should include preparation for more independent living.

Aftercare Support
You will play an active part in deciding your aftercare plan that will be produced following a thorough assessment of your on-going aftercare needs.

When leaving care, you will have a keyworker who will be your main point of contact for advice, information and accessing support.

You will be encouraged to keep in contact with your previous carers after you have left care.

After leaving care, you will receive on-going support from your local authority, which may also include financial support instead of benefits if you are 16 or 17 years old.

When receiving financial support from the local authority, you should be no worse off than if you were receiving benefits, and you will be supported to maintain an agreed and acceptable standard of living.

Wherever possible, you will be given more than one option for kind of accommodation you wish to live in.

You will be given support to lead a healthy lifestyle and to have any health problems addressed.

You will receive support to access education, training and employment opportunities, especially through your local careers services.

If you move to another area, you will be entitled to a similar level of throughcare and aftercare support and you will not face delays in gaining the support you need.

Resolving Difficulties
You will have the right to speak out if you are unhappy with any of your throughcare and aftercare arrangements, and you will be supported to make complaints or appeal decisions.

You will be able to request another keyworker if you do not have a good relationship with the one you have.

Any difficulties or complaints will be swiftly dealt with and you will receive appropriate feedback on how issues will be resolved.

Having Your Say
You will be given opportunities to have your say about your experience of leaving care, the quality of throughcare and aftercare support, and what services are needed for young people leaving care.
